  • Information provided in this process is 100% confidential and all data is managed within a HIPAA-compliant environment.
  • Emotional Relief Fund typically does not request additional clinical information beyond the point of assessing eligibility for funding.
  • If approved for the scholarship, your child will have 12 months from the date of approval to utilize the approved funding. If the funds are not used and there is still a need, your family can reapply following this expiration.
  • No clinical decisions will be made based on your application for an Emotional Relief Fund scholarship and no diagnosis will be assigned by Emotional Relief Fund. The clinical diagnosis of your teen will be left solely to your teen’s treating provider(s).
  • We strive to provide funding for as many individuals as possible and ensure that those receiving funding are committed to treatment. Therefore, if your child misses two sessions with less than 24 hours notice provided to the Clinician (i.e. is a “no show” more than once), funding will be terminated and no additional funding will be available.
  • Emotional Relief Fund will provide up to $3,000 per child per lifetime. We hope that this will provide adequate funding to you and your family to allow you to be able to budget and save for additional expenses beyond this level, and will ensure that we can help other families in their time of need.
  • Payment will be made directly to the treating Provider(s) within 45 days of billing receipt by Emotional Relief Fund.
  • By completing this form, you are attesting that the answers provided are true to the best of your knowledge. Any falsification of information will result in the immediate disqualification from the Emotional Relief Fund scholarship review process and/or receipt of funding from Emotional Relief Fund, even if some funds have already been paid to a provider.
